Self Assessments

Self-assessments are designed to achieve two objectives. The first is to document adequate maintenance of the Quality Care Pharmacy Program (QCPP) Standards and the second is to introduce the concept of Continuous Quality Improvement (CQI).

Self-assessment Forms have been provided to form the basis of an annual internal report on the QCPP in an accredited pharmacy.  These forms consists of four sections:

  • Section 1 covers the Maintenance of QCPP Standards;

  • Section 2 covers the Introduction of New QCPP Standards;

  • Section 3 relates to CQI Management and Reporting; and

  • Section 4 records the annual Program membership renewal.

Each of the forms relate to the first and second anniversary of the initial accreditation and the first and second anniversary of the re-accreditation. Right click on the links below to view one or all of Self Assessment reports in PDF format:
